Chronic hepatitis B remains one of the world's leading causes of liver-related illness, affecting more than 250 million people globally and contributing to substantial rates of cirrhosis, liver failure and liver cancer. While existing antiviral therapies can effectively suppress viral replication, they rarely allow patients to discontinue treatment completely. New research, however, is raising hopes that this long-standing challenge may eventually be overcome.

Results from international clinical trials presented at a major scientific meeting in Barcelona, Spain, and published in the New England Journal of Medicine have highlighted the potential of an investigational medicine known as bepirovirsen. Researchers reported that approximately one in five participants with chronic hepatitis B achieved what specialists describe as a “functional cure”, enabling them to stop treatment while maintaining long-term control of the virus.

Unlike complete eradication of the infection, a functional cure refers to a state in which viral activity becomes so low that the body's immune system can keep the disease under control without the need for continuous medication. This outcome has long been considered a major objective in hepatitis B research.

The studies involved more than 1,800 patients receiving treatment for chronic hepatitis B. Participants were administered weekly injections of the experimental therapy alongside their standard antiviral medication over a six-month period. Investigators observed that a proportion of patients maintained undetectable levels of viral activity even after discontinuing both the investigational treatment and their routine therapy.

Researchers noted that individuals who entered the trials with lower levels of hepatitis B surface antigen appeared more likely to achieve sustained viral control. This finding may help identify which patients could benefit most from future treatment strategies.

A New Approach to Viral Suppression

Bepirovirsen is designed to target the genetic material of the hepatitis B virus directly. By interfering with viral replication and reducing the production of key viral proteins, the treatment aims to expose the infection to the immune system and improve the body's ability to suppress it naturally.

Scientists believe this mechanism represents a significant advancement beyond traditional antiviral medicines, which primarily focus on limiting viral multiplication but generally do not eliminate the virus's ability to persist within liver cells.

According to global health authorities, hepatitis B continues to be a major public health concern. The disease is transmitted through contact with infected blood or bodily fluids and can also be passed from mother to child during childbirth. Vaccination remains the most effective preventive measure and has dramatically reduced infection rates in many countries.

Remaining Questions Before Widespread Use

Although the findings have been described as highly encouraging, experts emphasise that further research is necessary before the therapy can be considered a standard treatment option. Questions remain regarding the durability of the response, the factors that determine which patients benefit most, and the effectiveness of the medicine in people with more advanced liver disease.

The trials did not include some higher-risk patient groups, including individuals with cirrhosis, meaning additional studies will be required to assess safety and efficacy across a broader population.

Reported side effects were generally manageable, with the most common being mild reactions at the injection site and temporary increases in liver enzyme levels. Researchers continue to monitor participants to determine how long viral control can be maintained after treatment cessation.

Implications for Future Hepatitis B Care

The emergence of therapies capable of achieving functional cure could transform the management of chronic hepatitis B. For decades, many patients have relied on lifelong antiviral medication to prevent disease progression and reduce the risk of serious liver complications.

While regulatory reviews and additional clinical investigations are still underway, the latest findings represent an important milestone in the search for more effective and potentially finite treatment options. If future studies confirm these results, patients living with chronic hepatitis B may one day benefit from therapies that offer sustained viral control without the burden of continuous medication.
